Description This form is a booklet used to list the buildings, housing units, households, and the Institutional Living Quarters within an EA. This form also records other important information such as the name of household heads and name and type of institutions and their addresses, population totals, and counts of males and females.
Table of contents CONTENTS OF CPH FORM 1

CPH Form 1 is a four-page listing booklet. The first page of this form contains four sections, namely:

Title Panel: Contains the form type, authority and confidentiality clauses, office name and the title of the census and CP form, serial number, page number, PSA approval number, and expiration date.

Identification Panel: Consists of certification portion and geographic identification.

Listing Record: Contains 10 columns which consists of the line number, day of visit, BSN, HUSN, HSN, institutional serial number (ISN), name of household head or name/type of institution and address, total population count, population count of male and female and the remark/s portion.

Page Totals: This portion is found at the bottom of the form. It is used to record the total number of buildings, housing units, households and institutions, and the total male and female population in the households and institutions listed on the page.

The second, third, and fourth pages of this form also consist of Listing Record and Page Totals.

The listing record of page 1A has 10 rows for the listing of buildings, housing units, households, and ILQs.

The succeeding pages (1B, 1C, and 1D) contain listing records, each with 15 rows. One booklet of CP Form 1 has 55 rows in all.

Description This questionnaire was used to gather data on the basic demographic, social, and economic characteristics of the household population and some characteristics of their housing units. It also collects data on registration of deaths in the last two years among household members.
Table of contents CP Form 2 is a four-page questionnaire which served as the record of the basic characteristics of the household population and the housing units they occupy. The first page (2A) of this questionnaire contains the following:

1. Title Panel
2. Certification Panel
3. Geographic Identification Panel
4. Interview Record
5. Remarks Portion

The second and third pages (2B and 2C) contain the Population Census Questions. The fourth page (2D) contains the questions on housing, as well as questions on registration of deaths in the last two years among household members. Below is the list of questions asked in the CP Form 2.

Population Census Questions

For all persons:
P1 - Name of the household member
P2 - Relationship to the household head
P3 - Sex
P4 - Date of birth
P5 - Age
P6 - Whether a member's birth was registered at the LCR Office
P7 - Copy of birth certificate
P8 - Marital status
P9 - Religious affiliation

For all persons 5 to 24 years old:
P10 - School attendance

For all persons 5 years old and over:
P11 - Literacy
P12 - Highest grade/year completed

For all persons 15 years old and over:
P13 - Graduate of technical/vocational course
P14 - Technical/vocational course obtained
P15 - Overseas worker
P16 - Usual activity/occupation

B1 - Type of building
B2 - Construction materials of the roof of the building
B3 - Construction materials of the outer walls of the building/housing unit
H1 - Fuel for lighting
H2 - Source of water supply for drinking
H3 - Source of water supply for cooking
H4 - Tenure status of the housing unit/lot

This form is used to record the characteristics of each barangay, facilities in the area or within some specified distance from the area, and travel information from town/city proper to the barangay, kind of establishments, informal settlers, presence of relocation areas, and in-movers.

CPH Form 5 is a four-page questionnaire used to record the barangay characteristics and facilities.

Part III - Informal Settlers

This part of CP Form 5 pertains to the presence of some informal settlers in the barangay. Informal settlers are households who reside in areas such as esteros/creek/waterways, river bank/shoreline, along railroad, garbage dumpsite, under the bridge, sidewalk or easement of roads and highways, other danger areas such as cliff and cemetery, government lands without legally recognizable claims to the land, and private lands which they do not own.

Part IV - Presence of Relocation Area

This part of CP Form 5 determines whether or not there is a relocation area in the barangay whether temporary or permanent/resettlement.

Part V - In-Movers

This section intends to determine whether the barangay is one of the destinations of families displaced by natural or man-made calamities such as the following:
a. Typhoon
b. Other natural calamity/disaster
c. Peace and order problem?
d. Reasons other than natural calamity, disaster or peace and order problem

Description The 2012 Philippine Standard Occupational Classification (PSOC) was utilized for coding of usual occupation. Description The 2008 Philippine Standard Classification of Education (PSCED) was utilized for coding the degrees/courses of those who completed post secondary. technical-vocational education and college education. It is a detailed classification of all educational levels in the Philippine educational system. It aims to integrate the different classification schemes being used by the various agencies, for the purpose of standardizing and facilitating the compilation of education statistics as basis for educational planning, manpower training, labor market studies and other related activities.
