Description CP Form 1 - Listing Booklet

A booklet that was used to list the buildings, housing units, households, and institutional living quarters (ILQ) within an enumeration area (EA) or barangay. This form was also used to record other information such as address, total population and number of males and females.
Table of contents CP Form 1 is a four-page listing booklet. The first page of this form contains four horizontal panels, namely:

1. Title: Contains approval number from the National Statistical Coordination Board (NSCB), the name of CP form, serial number, page number, and authority and confidentiality clauses.

2. Identification: Consists of geographic identification, number of booklets of CP Form 1 used in the listing of an enumeration area (EA), and certification portions.

3. Listing Record: This is where you will record the characteristics of listing units in your EA.

4. Page Totals: This portion is found at the bottom of the form where you will record the total number of listing units (building and housing units), total number of households and institutions, and the total male and female population in the households and institutions listed on the page.

The second, third, and last page of the booklet contain the listing and page totals panels.

Description CP Form 2 - Household Questionnaire

A four-page questionnaire that was used to record information about the households. This form was used to gather information on selected demographic and socio-economic characteristics of the population and some information on housing characteristics.
Table of contents CP Form 2 is a four-page questionnaire which, when completed, will serve as the record of the characteristics of the household population as well as their housing characteristics.

The first page of the questionnaire shows the title of the questionnaire, the particulars about the geographic location of the household to be interviewed, and the interview record. A certification by the EN and his/her supervisors regarding the veracity and completeness of the data collected by the EN and reviewed by the supervisors are likewise to be recorded on this page. Also, at the bottom of the page are definitions of household and household membership to serve as guide in listing all the members of the household.

The second and third pages of the questionnaire contain the Population Census Questions. It consists of 14 columns containing the questions on demographic, social and economic characteristics of the household. From these pages, the following data are to be collected for POPCEN 2007:

b. Relationship to Head
c. Sex
d. Date of Birth
e. Age
f. Birth Registration
g. Marital Status
h. Attendance in School
i. Current Grade/Year in School
j. Place of School
k. Highest Grade/Year Completed
l. Overseas Worker
m. Usual Occupation
n. Place of work

The bottom of these pages contain the questions on the household size, specifically on whether there are household members not yet listed and whether there are six household members or more, and a space for remarks.

The fourth page contains the questions for housing characteristics such as contruction materials for roofs and outer walls, and tenure status of the lot, as well as questions for households in temporary relocation area, codes for relationship to household head, and codes for grade/year currently attending in school and highest grade/year completed.

Description CP Form 7 - Household Self-Administered Questionnaire

This is a 12-page questionnaire that is similar to CP Form 2 except that it has instructions on how to accomplish the form. All oddnumbered pages contain the similar information found in CP Form 2 while all the even-numbered pages contain instructions on how to fill out each column item.

Description CP Form 4 - Institutional Population Questionnaire

A four-page questionnaire that was used to record the characteristics of institutional population residing in ILQ like hotels, hospitals, home for the aged, and others.
Table of contents A single CP Form 4 can accommodate 12 persons. The cover page of this questionnaire contains the geographic identification and the certification portion, and the type of institutional living quarter. While the inside pages consist of the following items gathered from the institutional population.:

Description CP Form 5 - Barangay Schedule

This is a four-page questionnaire, which when completed, will serve as a record of the physical characteristics of each barangay, kinds of establishments and service facilities in the area or within some specified distance from the area, and information on informal settlers.
Table of contents The first page of the CP Form 5 shows the title panel containing the NSCB approval number and the expiration date, title of the questionnaire, serial number, and authority and confidentiality clauses. The first page also contains the particulars about the geographic location of the barangay to be visited and the interview record. A certification by the TS and his/her supervisors regarding the veracity and accuracy of the data collected by the TS are likewise to be recorded on this page.

Part I of CP Form 5, which covers the first and the second pages of the questionnaire, contains questions on barangay characteristics and facilities.

Part II, which covers the third and fourth pages of the questionnaire, contains questions regarding the kinds of establishments within and outside the barangay. On the lower part of the fourth page is Part III which includes a question about households in squatter areas (informal settlers).

Description CP Form 8 - Institutional Population Self-Administered Questionnaire Instructions

This is an eight-page instruction on how to accomplish CP Form 4. It also contains the message of the Administrator about the rationale of the census undertaking, basic information on census such as reference period, confidentiality of census data, whom to include in the enumeration, how census data will be processed, concepts and definitions, samples of properly filled out sections of CP Form 4 to serve as guide for the respondent, and contact information.

Description CP Form 10 - EN's Accomplishment/Progress Monitoring Report

This form is designed to provide information about the EN's daily output or are the number of buildings and housing units, total number of males and females, total population actually enumerated during the period, and the corresponding number of households, as well as the the number of institutional living quarters and the number of male and female institutional populations.

Number of every sheet in consecutive order to account all CP Form 10 that the EN had submitted to his/her TS.

SMS Receipt Number
This portion was filled out by CAS when the totals of each page of CP Form 10 had been sent to the Central Office through text messaging. An acknowledgment of receipt was made by the Central Office by sending the “SMS Receipt Number”.

Date
The month and day in the boxes provided at the upper left hand side the CP Form 10 was filled out. Two-digit code for the month and day (for example, August 22 is written as 08-22).

Geographic Identification
Province, city/municipality and barangay names and the corresponding geographic codes, and EA number.

EA Status
Code “1” if the enumeration for the EA is still ongoing or “2” if already completed. If an EA is completely enumerated, an entry of “00” for day of visit after the last row that is filled out in CP Form 1.

Column 1 - CP Form 1 Serial Number and Page
The serial number and the page letter of the completed page of CP Form 1 corresponding to this line. For instance, if accomplished CP Form 1 has a serial number 000008, the entry in the first line should be 000008 1A; on the second line, 000008 1B; on the third line, 000008 1C; and 000008 1D on the fourth line.

Column 2 (A) - Number of Buildings
The total number of buildings from the box labeled Total with letter A below it in Column 2 of CP Form 1.

Column 3 (B) - Number of Housing Units
The total number of housing units from the box labeled Total with letter B below it in Column 3 of CP Form 1.

Column 4 (C) - Number of Households
The total number of households from the box labeled Total with letter C below it in Column 4 of CP Form 1.

Column 5 (D) - Number of Institutions
The total number of institutions from the box labeled Total with letter D below it in Column 5 of CP Form 1.

Column 6 - Cumulative Total
This is the total household population listed in the EA since the start of enumeration in the EA. This will be filled out only after filling out Columns 7 to 9. To get the cumulative for the current line, add the page total in Column 7 (E) to the last cumulative total or entry in Column 6 appearing on the previous line.

Columns 7 (E), 8 (F), and 9 (G) - Household Population
The total household population from the box labeled Total Household Population with letter E below it. Do the same for the male (F) and female (G) household population in Columns 7, 8 and 9 of CP Form 1.

Columns 10 (H), 11 (I), and 12 (J) - Institutional Population
The total institutional population from the box labeled Total Institutional Population with letter H below it. Do the same for the male (I) and female (J) institutional population in Columns 7, 8 and 9 of CP Form 1.

Column 13 - Number of Callbacks
The number of households listed on the particular page of CP Form 1, which were not successfully interviewed. These are the households scheduled for callbacks on a later date in Column 10 vis-à-vis Column 1 (indicator) of CP Form 1.

Column 14 - Date Accomplished
The latest date when a particular page of CP Form 1 had been accomplished. Refer to Column 1 of CP Form 1.

Row Totals
For every CP Form 10 submitted, this is the sum of all entries in Column 4 for the Total Number of Households in the box labeled Total with letters HH below it; Column 7 for Total Household Population, Columns 8 and 9 for the male and female household population in the box with letters HP, HM, and HF below the boxes, respectively; Columns 10 (IP), 11 (IM) and 12 (IF).

Total in Column 7 should be equal to the last cumulative total in Column 6. The sum of Columns 8 and 9 should also be equal to the total in Column 7. The sum of Columns 11 and 12 should be equal to the total in Column 10.

Name and signature of the EN on the line “Prepared by:”

Name and signature of TS and the date after he/she has reviewed or verified the entries in the form.
