The answer will be evaluated and rated in the following manner:
1 - Satisfactory
2 - Not satisfactory
3 - Not answered independently
4 - No answer

Specific instructions in evaluating responses:
Item 4 = Highest educational attainment
a. Assign a score of “satisfactory” (code 1) if one box is checked and an appropriate highest grade completed is specified.
b. Assign a score of “satisfactory” (code 1) if more than one box is checked but appropriate highest grade completed is specified in one of the lines corresponding to these boxes.
c. Assign a score of “not satisfactory” (code 2) if more than one highest grade completed is specified.
d. Assign a score of “not satisfactory” (code 2) if none of the boxes is checked, even if the highest grade completed is specified.
e. Assign a score of “not answered independently” (code 3) if there is a remark indicating that someone has assisted the respondent in filling out this item.
f. Assign a score of “no answer” (code 4) if the item has been left blank.
