Item 3 = Date of birth
a. Date of birth must be complete - in month, day and year. Assign a score of “satisfactory” (code 1) if the date of birth is complete and in any order, and “not satisfactory” (code 2) if it is incomplete.
b. Date of birth written in numerals are acceptable, hence, assign a score of “satisfactory” (code 1).
c. Assign a score of “satisfactory” (code 1) if the day comes first before the month and year.
d. Abbreviations for months and the use of last two digits for year are acceptable. Hence, assign a score of “satisfactory” (code 1).
e. Assign a score of “not answered independently” (code 3) if there is a remark indicating that someone has assisted the respondent in filling out this item.
f. Assign a score of “no answer” (code 4) if the item has been left blank.
