c. Assign a score of “satisfactory” (code 1) if there is no street or purok name.
d. Assign a score of “not satisfactory” (code 2) if the barangay, town/city and province are incomplete or interchanged.
e. Assign a score of “not answered independently” (code 3) if there is a remark indicating that someone has assisted the respondent in filling out this item.
f. Assign a score of “no answer” (code 4) if the item has been left blank.
