Interviewer instructions
Write on the lines the names and characteristics of all the members of the household.
Ask the respondent the question “How many members are there in this household?” While asking this question, be sure to explain to the respondent the meaning of household and household members. If for example, the respondent answers six, check line number 06. Later, this will serve as guide in determining whether or not all members of the household have been listed in Column 2.
Ask the respondent to give the names of the persons who usually live in the household. Make sure that the names of all the household members are listed down. No one should be missed.
List the names of the members of the household in the following order:
a) Head
b) Wife/spouse
c) Unmarried children of the present marriage from oldest to youngest, regardless of sex
d) Unmarried children of the head by previous marriage from oldest to youngest, regardless of sex
e) Unmarried children of the spouse by previous marriage from oldest to youngest, regardless of sex
f) Ever-married children and family, (son-in-law or son first)
g) Parents, grandparents, brothers and sisters, parents-in-law, grandparents-in-law and other relatives of the head or wife.
h) Boarders (including relatives who are mere boarders)
i) Domestic helpers (including a relative employed as domestic helper)
j) Nonrelative
In writing the name of a member of the household, write the surname or family name first, a comma and then the first or given name and initial, if given. If the surname of a member is the same as that of the immediately preceding one, simply draw a horizontal line and a comma (see Example 1) in place of this surname and write his first name. In communities where members do not carry surname, write the name as given and indicate the appropriate remarks.
After listing the names of the household members, encircle the line number of the respondent. If there are two or more respondents, encircle only the line number of the one who answers most of the questions.

Line number - a precoded number printed at the first column of the questionnaire.
This number automatically identifies each member of the household. Thus, the first number is entered on the first row with line number 01; the second number on the second row with line number 02, and so on.
Every FLEMMS 2003 questionnaire is provided with 15 lines or rows.
