Question pretext
-If ligation is used in combination with any other method, encircle code 6 for ligation/female sterilization.
-If using any method which requires supply/service (codes 1-7) and any method which does not require supply/service (codes 8-13), encircle the method which requires supply/service (codes 1-7). An exception is a lactational amenorrhea method (LAM) user who meets the three conditions but at the same time is using a back-up method which requires supply/service. In this case, encircle code 11 for LAM.
-If the woman is ligated or using IUD and her partner has had a vasectomy, consider the method used by the woman as the current method. Encircle the corresponding code.

Interviewer instructions
Calendar/rhythm/periodic abstinence - the couple avoids sexual intercourse on certain days of the woman's menstrual cycle (around the time of ovulation) to avoid pregnancy. The woman calculates and marks on the calendar the days when she is likely to conceive to remind the couple not to have sexual relations on those days. Likewise, a couple is having rhythm if they use a "rule" to determine which days not to have intercourse, such as no intercourse from day 8 to day 21 of the menstrual cycle. Periodic abstinence is not the same as prolonged abstinence where the couple stops having sexual relations for months at a time to avoid pregnancy without regard to the woman's monthly cycle.
Condom - the condom is a rubber or latex sheath used by male partner during the intercourse to prevent the sperms from entering the uterus. Condoms are most commonly known as rubbers.
Diaphragm, foam, jelly, cream - these are female methods which are used in the vagina. The diaphragm is a rubber device placed at the entrance to the uterus before the intercourse to prevent sperm from entering the uterus. An aerosol foam is a spermicidal foam inserted in the vaginal canal before the sexual intercourse.
Injectable - an injection that is normally given every two or three months and is also known as depo-provera, noristerat or DMPA (depot medroxy-progesterone acetate). It prevents ovulation by stopping the pituitary glands to release the egg from the ovary.
Intra-uterine device (IUD) - a small plastic or metal device inserted in the uterus (by a doctor/nurse) and remains there until removed by the doctor/nurse or accidentally expelled. The IUD is not to be confused with chemical preparations or devices (such as the diaphragm) which are inserted in the vaginal canal shortly before the sexual intercourse.
Lactational amenorrhea method (LAM) - is a child spacing method promoted by the DEpartment of Health (DOH) that requires full and regular breastfeeding which results in the delay of the mother's ovulation. LAM is a temporary method that can be used until the infant is six months old.
Ligation or female sterilization - tubal ligation is permanent method to avoid pregnancy by means of tying or cutting the fallopian tube to prevent the egg from flowing to the uterus.
Mucus/billings or ovulation - the woman checks the consistency of vaginal mucus to determine the time of ovulation.
Pills - are tablets taken orally each day for at least 20 days of the menstrual cycle, suppressing ovulation. They are not to be confused with foam tablets which are inserted in the vaginal canal shortly before the sexual intercourse to prevent conception by killing the sperms.
Thermometer or basal body temperature (BBT) - the time of ovulation is gauged by observing fluctuations in the woman's temperature during the menstrual cycle.
Vasectomy or male sterilization - vasectomy is a relatively minor operation (compared to ligation) done on men for contraceptive purposes. it is a permanent method performed on men by means of tying or cutting the vas deferens so that the sperm will not mix with the semen.
Withdrawal - refers to the voluntary removal of the male sex organ just before the climax is reached during sexual intercourse.
