Subsection H1. Are you or any member of your family a member of any of the following health insurance plan(HIP)?
d. Community/Cooperative Health Financing Scheme - coverage based on voluntary contributions made as member of an organization in the community or place of work (not employer sponsored) or cooperative schemes. Premiums are paid by members to a common fund, either managed directly by an organization or by an outside manager. It may be any of the following forms;
- cooperative contracts with a provider or facility or Health Maintenance organization (HMO) to provide free or subsidized care to members and beneficiaries (UP Cooperative/Davao Health Service Cooperative); and
- members in a community voluntarily group together for the purpose of providing assistance in times of health need; contribute to a common fund and source from this fund for assistance at the time of need; or use this fund as seed money to support health worker, pharmacy, etc.

Description
The whole concept of membership in a Health Financing Scheme/Health Insurance Plan (HIP) involves contribution to a common fund, in the form of premiums or contributions, entitling each member to a specified benefit, with contributions and premiums not being returned when no benefit was utilized (except in some dividend paying funds). The idea of pooling funds also entails some sharing of risk, where the healthy (those who did not avail of benefits) subsidize or help pay for the needs of the sick (those who avail of benefits). At the point of service, use may be free, discounted, or to be fully paid for reimbursement (full or partial) depending on the type of plan or coverage.
