If the woman's main reason is not listed as a response, write her response in the “OTHER” category and encircle “96”. If the woman does not know at all why she will not use contraception, encircle “98” for “DON'T KNOW”.

3 - Nausea, vomiting, or headache
4 - Breast tenderness or sensitivity
5 - Weight gain or loss
6 - Upset stomach or diarrhea
7 - Depression or irritability
8 - Loss of interest in sex
9 - Skin problems
10 - Itchiness or pain in genital area
11 - Hypertension
12 - Anemia
13 - Cause cancer abd other diseases
14 - Cause abortion
15 - Deformed babies
16 - Painful procedure
17 - Sexual disability
18 - Rods can move around
96 - Others
