For each pregnancy that the woman had, ask if it resulted in single birth or multiple births. In case of multiple births, be sure to record the twin (or triplet, and so on) on separate lines. Use one line for a pregnancy that ended before full term, even if it is a “multiple pregnancy”.
Encircle “1” if the pregnancy resulted in single birth and “2” if it is multiple births. In case the pregnancy was lost before full term and it cannot be ascertained from the fetus whether it is a single or multiple birth, encircle “1” for single.
