Questions and instructions
This question must be asked independently of the information on the Household Questionnaire. Even if the respondent was already asked her age while completing the Household Questionnaire, this question must be asked again: her date of birth on the Individual Woman Questionnaire.
If the respondent knows her date of birth, write it in the appropriate boxes for MONTH and YEAR. Convert the month into numbers. For this, January is “01”, February is “02”, March is “03”, and so on. Write the complete year of birth in the four boxes for YEAR e.g., 1954 would be written “1954”.
If she does not know her month of birth, encircle “98” for DON'T KNOW MONTH and ask her for the year of her birth. Try under all circumstances to obtain at least the year of birth. If the respondent is unable to provide this information, ask if she has any identification card, or birth or baptismal certificate that might give her date of birth. Only when it is absolutely impossible to even estimate the year of birth should you circle “9998” for DON'T KNOW YEAR.
