If not satisfied, ask reasons for dissatisfaction an encircle the code/s of reason/s she mentioned (E09). If the respondent answers for other members, ask him or her if he/she knows about the uti of the other member(s) and if that member is satisfied or not. If in case the respondent does not know about it, encircle Code “8” for DON'T KNOW.
