Description
Type of PhilHealth member - the type of PhilHealth membership any or all members of the houshold is/are part/s of
The categories to choose from are as follows:
1. Indigent - refers to a person who has no visible means of income or whose income is insufficient for the subsistence of the family, as identified based on specific criteria set by the corporation in accordance with the guiding principles set forth in Article I of the National Health Insurance (NHI) Act.
2. Private sector employee-an employee who render services in any of the following:
· Corporations, partnerships, or single proprietorships, no government organizations, cooperatives, nonprofit organizationss, social, civic, or professional or charitable institutions, organized in the Philippines, whether based on the Philippines or abroad;
· Foreign corporations, business organizations, with quasi-state status based in the Philippines or abroad which entered into an agreement with the Corporation to cover its Filipino employees in the NHIP,
· a foreign business organizations based abroad with agreement with the Corporations to cover its Filipino employees in the NHIP;
· Household employers availing services of household helpers.
· Government employee - refers to an employee of the government, whether regular, casual or contractual who render services in any of the government branches, military or police force, political subdivisions, agencies or instrumentalities, including government owned and controlled corporations, financial institutions with original charter, Constitutional Commissions, and is occupying either in an elective or appointive position, regardless of status of appointment.
3. Individually paying member- refers to a person who pays required contribution in full.
4. Nonpaying members- refers to:
· A member of the GSIS or SSS who has reached the age of retirement or who has retired on account of disability prior to the effectively of the law on March 4, 1995; or
· A pensioner of the GSIS or SSS prior to the, effectively of the law on March 4, 1995 or
· A member who has reached the age of retirement as provided for by the law and has paid at least 120 monthly contributions.
