The whole concept of membership in a Health Financing Scheme/Health Insurance Program involves contribution to a common fund, in the form of premiums or contributions, entitling each member to a specified benefit, with contributions and premiums not being returned when no benefit was utilized (except in some dividend paying funds). The idea of pooling funds also entails some sharing of risk, where the healthy (those who did not avail of benefits) subsidize or help pay for the needs of the sick (those who avail of benefits). At the point of service, use of the insurance plan may be free, discounted, or to be fully paid for reimbursement (full or partial) depending on the type of plan or coverage.

The choices are:
A - PhilHealth
B - HMO/private insurance
C - Local government unit, community, or cooperative

The detailed description of the health care financing scheme or any Health Insurance Plan are as follows:
1. PhilHealth - the national health insurance program financed by compulsory contributions made by employees and employers to the Government Service Insurance System (GSIS) and Social Security System (SSS) for the sole purpose of providing for financial assistance in case of hospitalization.
Considered members of PhilHealth are:
a) salaried government employees including temporary, emergency, casual, and contractual appointees;
b) currently employed SSS members including registered self-employed; and
c) retired or separated employees who opted to continue membership within six months from date of separation by paying his contribution plus the employer's counterpart.
2. Employer-based HMO - coverage with a Health Maintenance Organization (HMO) as part of employment. HMO usually offers outpatient benefits in addition to inpatient or hospital care. Consultations or care can only be sought from HMO owned or accredited facilities. Members are usually asked to carry a card issued by the HMO. The employer may pay all or part of the premium.
An example of this is the Philippine Airlines Departments Medical Plan (PDMP). It serves only PAL employees and dependents.
3. Private Health Insurance Company - coverage with commercial indemnity insurance on a voluntary basis. Private insurance usually works on a reimbursement basis and for specific types of benefits, that is, hospitalization. Expenses incurred during hospitalization for example are presented to the insurance company after the confinement and reimbursement are made depending on the type of plan or coverage. Coverage may have been sourced directly from an agent and premiums are paid solely by the member of the household.
4. Community/Cooperative Health Financing Scheme - coverage based on voluntary contributions made as member of an organization in the community or place of work (not employer sponsored) or cooperative schemes. Premiums are paid by members to a common fund, either managed directly by an organization or by an outside manager. It may be any of the following forms:
• cooperative contracts with a provider or facility or Health Maintenance Organization (HMO) to provide free or subsidized care to members and beneficiaries (UP Cooperative/Davao Health Service Cooperative); and
members in a community voluntarily group together for the purpose of providing assistance in times of health needs; contribute to a common fund and source from this fund for assistance at the time of need; or use this fund as seed money to support health worker, pharmacy, etc.
5. Others - if the health care financing scheme of any member of the household cannot be classified in any of the four specified categories.
