For each facility, a box is provided for writing the code of the health facility visited during the last six months.
For all health facilities with a “YES” response in AOl, write their corresponding codes and the name of the health facility in the boxes provided. The first health facility with a “YES” response in AOl should occupy the first rectangular panel in A03, the second facility occupies the second panel and so on.
Starting from the health facility in the first panel, ask from the respondent the type of service he/she or any member of the household availed when he/she or the member visited that health facility.
Read out each type of service to the respondent to help him/her recall the services availed from the health facility during the last six (6) months. For each service availed, encircle code “1” in the appropriate row. If the service was not availed, encircle code “2”. Ask the services availed for the next type of health facility, if any. If no other health facility was visited, go to BOl.
It is possible for the same service to have been availed of from different facilities, as for example, when a woman gets family planning advice from a doctor of a private hospital but gets her supplies of pills from the barangay health station. In this case, category e (family planning) will have a code of “1” for both private hospital and barangay health station.

Services utilized - the type of service the respondent or any member of the household availed of in the health facility he/she/they visited during the last six months.
The categories of services that may have been availed of are listed below with examples under each:
a. Treatment when ill or injured
- dressing of open wounds
- dispensation of drugs/medicines
- injections
- surgery
- consultation or confinement due to an illness
b. Routine check-up (excluding prenatal and postnatal check-up
- reading of body temperature
- reading of blood pressure
- check-up of other body functions such as heartbeat
- confinement in hospital/clinic to undergo executive check-up
c. Laboratory services
- x-ray, microscopy, sputum exam, and the like
- analysis of specimen such as blood, urine, stool, mucus, pap smear, acetic wash, and pregnancy test
d. Immunization services - vaccination for the prevention of various kinds of illnesses such as:
* poliomyelitis
* tuberculosis
* diphtheria, among others
e. Family planning
- advice on birth spacing or birth control methods
- availment of contraceptive supplies
- insertion of IUD
- sterilization
f. Health and nutrition education
- advice on eating habits that are nutritionally sound, practical, and consistent with individual needs and available food resources
- advice on the prevention and control of disease
g. Prenatal, delivery, and postnatal
- attendance in child delivery
- prenatal, postnatal care
- advice on breastfeeding
- child care of newly born babies
The code corresponding to each health facility are as follows:
1 - Barangay health station
2 - Rural health unit/urban health center
3 - Municipal hospital
4 - District hospital
5 - Provincial hospital
6 - Regional hospital/public medical center
7 - Private clinic
8 - Private hospital
9 - Others
