If the person usually lives in the household, in Column 5, encircle “1” for YES. Otherwise, encircle “2” for NO.
If the household head has more than one spouse living in the same household, list down the name of the first spouse and their children, followed by the second spouse and their children, keeping the nuclear family together, if possible.
In listing household members, enter the surname first, followed by a comma, and then the first/given name and middle name initial, if given. If the surname of a member is the same as the one immediately preceding him/her, just draw a horizontal line and a comma (______,) in place of his/her surname and write the first name of the person.
In some parts of the country, people do not have surname. In such case, just write the name as given.
